1. Completed and signed Cadet Application and Agreement, Form NSCADM 001. Please print on one sheet, front and back.
2. Completed Report of Medical History, Form NSCADM 020. Please print on one sheet, front and back.
3. Completed Report of Medical Examination, Form NSCADM 021. Please print on one sheet, front and back.
4. Signed Release of Liability from the Naval Diving and Salvage Training Center. Click on the link below; sign and return
ONLY the last page of the document with the application packet. The rest of the document may be retained
for your reference.

5. Copy of the applicant's most recent report card.
6. Enrollment fee (cash, check, or money order payable to USNSCC/Seawolf Division).
